Facility Evaluation Report

At approximately 09:45 AM Licensing Program Analyst Star Stevenson arrived unannounced to CONTINUE a required 1 year annual inspection multiple phones calls were made to Griffin Family representatives to make entry into the home and the phone number on file by licensee was not functioning. Griffin Family employee and general manager Gabby Echevarria was called and she arrived at 10:45 AM to help with inspection.

At approximately 11:15 AM administrator Elaine Villalobos return from taking two clients to medical appointments. I had Gabby and other staff try to contact the facility with the phone number on file and they did not have success. Gabby reported that, "they been having trouble with ATT or Verizon for a month or so and we need to get that fixed" Administrator Elaine Villalobos also reported that the facility phone number has not worked since the facility changed phone providers.(Type B citation issued)

Recent tall brown grasses and weeds and over-hangning branches were observed to be cut, trimmed and cleared and a safer environment.

At approximately 11:00 AM five (5) of 5 staff files were observed and two (2) of 5 were missing documents, with staff 1 (S1) needing evidence of work physical and staff 2 (S2) needing evidence of TB clearance and work physical (Technical Violation issued)

Three (3) of 3 client files were reviewed and a technical advisory to ensure that all clients have Consent for emergency medical treatment (LIC627C) signed by their responsible parties. Clients are vendored through North Bay Regional Center.

A current Surety Bond was obtained during today's inspection and the licensee will send an updated LIC400 (Affidavit Regarding Client/Resident Cash Resources)
