Facility Evaluation Report

An office meeting was conducted today, 09/03/2025, in the Santa Rosa Regional Office. The following individuals were present in the meeting: Licensing Program Manager, Bethany Moellers, Licensing Program Analyst, Julie Florio, House Manager, Edward Gadia, and House Co-Manager, Lourdes Gadia. Licnesee/Administrator was unable to attend today's meeting. The Department has received some of the required documents to process a change of administrator to Edward Gadia. .

The purpose of the office meeting was to hold an Informal meeting to address the following areas of concern identified by the Department:
