Facility Evaluation Report
At approximately 12:45 PM, Licensing Program Analyst (LPA) Stevenson arrived unannounced to continue a Required 1 Year visit and met with Staff Member, Jiapel Wang, who called Licensee Guolian "Mark" Zhang.
At approximately 1:15PM Licensee Gulaian "Mark" Zhang arrived to assist in the continued required annual inspection.
Facility is an Adult Residential Home that provides care and assistance for Adults with Mental Health Diagnoses.
Upon arrival, LPA was informed there were currently 3 clients in care and 1 staff member on site. The one on-site caregiver that was available when I arrived admitted to having difficulty with the English language and the licensee was told of the requirement CCR 80075(h) that "there shall be at least one person capable of and responsible for communicating with the emergency personnel in the facility at all times" and a technical violation was issued.
At approximately 1:30 PM, Staff files were all found to be well organized, thorough and contained the required documentation. Staff have current first aid/CPR certification. Medication was observed to be centrally stored and secure.
Three (3) of three (3) client files were found to have all required documentation and Licensee was asked to ensure that the most current responsible party sign his clients Consent for Emergency Medical Treatment (LIC627C)
Facility's last emergency/disaster drill was February 2024. Facility's fire system was last inspected January 2024.
