At approximately 9:45 AM Licensing Program Manager (LPM) Star Stevenson arrived unannounced to perform a case management visit related to a self-reported incident received by the department on 03/10/2026 in which is was noted that client one (C1) was found with a left black eye.
LPA made observations, conducted interviews and obtained documents related to the incident.
Document was reviewed with House Manager Silva Rosario whose signature denotes receipt.
