Facility Evaluation Report
At approximately 10:35 AM Licensing Program Analysts (LPAs) Stevenson and Florio arrived unannounced to conduct an Annual Required inspection and were greeted by staff Aida Aquino who has Designation of Facility Responsibility (RP). RP called licensee Elpidio Punzalan who was made aware of todays inspection.
LPAs initiated a tour of the facility around 11:00 AM and made the following observations: Facility was a comfortable temperature, clean and without odors. Passageways were free from obstructions. Resident rooms were furnished per regulation. Water temperatures measured within 105 to 120F per regulation. Extra hygiene products and linens were available. Kitchen cabinet containing cleaning supplies and knives and medicines were observed to be locked. Facility has at least two days of perishable and one week of non-perishable foods which appeared to be of quality and stored per regulation. Medications were centrally stored and locked and a spot check confirmed them to be accurately recorded.
A closet door in the bedroom of client (C1) is not working and a technical advisory to develop a better/more robust closet door system was issued.
Interview and observation confirmed activities including day outings, piano, karaoke, patio deck with both sun and shade available.
Fire extinguisher was last inspected May 2025 and observed to be charged. Smoke and Carbon Monoxide detectors located throughout the facility were tested and operational. Most recent fire/disaster drill was conducted January 3th 2025.
