The inspector’s account

On 10/13/2025 at approximately 9:15AM, Licensing Program Analyst (LPA) Ali Deniz arrived unannounced to conduct a Required 1 Year visit and met with Program Director, Arnold Santiago. Facility is an Adult Day Program for Adults with Disabilities. Facility has an approved fire clearance and capacity for 45 Non-Ambulatory clients. Upon arrival, LPA was informed that there were 18 clients attending Day Program and 16 staff members on-site. Facility is a one-story building and provides in-person services, Monday through Friday from 8:30 AM - 2:30 PM. Facility is vendorized by North Bay Regional Center (NBRC).

At approximately 10AM, LPA reviewed the Facility's Staff Roster and found that all staff on-site were background cleared and associated to the facility per regulation.

At approximately 10:45AM, LPA and Program Director toured the building and grounds which was found to be clean and in good repair. LPA observed all walkways and exits to be unobstructed. All notices that are required to be posted have been posted and are in a highly visible area. LPA observed activity supplies for clients use. Chemicals and other items which could pose a risk to clients in care stored in locked room. Soap & paper towels were available in facility bathrooms. Facility kitchen, refrigerators and freezers were clean, and food was stored properly. The facility has hot water warning signs in all faucets accessible to residents.

Fire extinguishers were charged and last serviced 07/24/2025. Facility has a centrally hardwired sprinkler system which was last inspected 07/10/2024 and report was indicating the test are passed. There was enough lighting in all common areas and hallways. Medication is centrally stored and secure. Facility has 4 Vans for clients transportation.
