Facility Evaluation Report

At approximately 11:45AM, Licensing Program Analyst (LPA) Felias arrived unannounced to conduct a Case Management - Incident visit and met with Program Administrator, Arnold Santiago. The purpose of the visit was to follow up on a self-reported incident report that was submitted to Community Care Licensing (CCL).

Incident Report 1/SOC341 : CCL received incident report on 03/17/2025 and SOC341 on 03/18/2025. Reports state that on 03/14/2025 facility staff were informed by Client 1's (C1) responsible party that C1 was not at their baseline. C1's responsible party reported to facility staff that C1 said that someone touched the lower part of the their belly while on the transportation bus on 03/13/2025. Facility made all appropriate notifications per regulation.

LPA requested documents and conducted interviews.

Exit interview conducted. Copy of report discussed and provided to Program Director. Signature on form confirms receipt of documents.
