The inspector’s account

At approximately 8:40 AM Licensing Program Analyst (LPA) Star Stevenson arrived unannounced to conduct a case management visit due to an SOC341(suspected client neglect) for client (C1) received on 02/11/2026.

LPA was met by caregiver Maria Barriga who has Designation of Facility Responsibility (RP). RP called Administrator Higinia Alvis who arrived at approximately 9:15 AM to further assist with today's case management visit. Facility has a census of four(4) client and three(clients) were present and waiting to leave to their respective day programs. While client (C1) was already away at their day program.

LPA noted that the facility was calm, a comfortable temperature and without odors.

LPA conducted interviews, made observations and obtained documents.
