Facility Evaluation Report
At approximately 9:35 AM, Licensing Program Analyst (LPA) Star Stevenson arrived unannounced to conduct a required 1-year annual inspection and was greeted by caregiver with designation of facility responsibility, Maria Barriga. Licensee/Administrator Higina Alvis was called and advised to continue inspection with Maria Barriga. Facility is an Adult Residential Facility with three (3) Clients in care, all of whom were away at Day Program. Licensee is vendorized with North Bay Regional Center (NBRC).
At approximately 9:50 AM, LPA initiated a tour of the facility and observed the following: Facility is a one story home, was a comfortable temperature, and passageways were free from obstructions. Water temperature in clients' bathrooms measured within the allowable range of 105 to 120 degrees F per Title 22 regulations. LPA observed a supply of clean linens available to clients. However, LPA observed no paper towels available in facility bathrooms for clients. Clients' bedrooms were inspected and observed to have all of the appropriate furnishings as outlined in Title 22 regulations. Cabinets containing cleaning supplies and other items that could pose a risk were locked. LPA observed lockable facility door to laundry, storage, and staff bedrooms, as well as, separate locks on detergents above wash machine and dryer and separate locks on staff bedrooms. Caregiver was advised to ensure these were consistently locked when clients are present. Facility has at least two days of perishable food and one week of non-perishable foods, as well as an emergency water supply. Medications were centrally stored and locked.
LPA observed a supply of games and craft supplies for clients, as well as, a piano, televisions and radios. LPA was informed that facility staff and/or Licensee take clients on weekend outings of their choice and usually plan a yearly vacation or camping. Facility has internet available to clients in care and the phone was tested and operable. Licensee states that each client has their own internet access device. There is a shaded patio and seating area in the backyard with outdoor space for activities, including gardening. A rear bathroom was noted to have only 1 of 3 light bulbs working and caregiver was advised to replace 2 light bulbs to add light to an already older/darker home.
