Facility Evaluation Report
On 07/20/2026, Licensing Program Analyst (LPA), Jill Nakagawa unannounced at A Place of Grace - Moosup for the purpose of conducting a Case Management inspection regarding clients and the accessibility of food and facility staffing.
LPA was met at the door by Administrator June Price to discuss. LPA and Administrator reviewed incidents of clients requesting foods that were not on the menu or recommended alternatives. A re-training of staff was done to inform staff of procedures for engaging with clients and protecting their personal rights.
Additionally, Administrator reviewed staffing. LPA found that all shifts were appropriately covered by the required number of DSPs and Lead RBTs.
No deficiencies were found at the time of inspection. No citations issued.
