The inspector’s account

Licensing Program Analyst (LPA) Jill Nakagawa arrived unannounced to A Place of Grace - Moosup to conduct an inspection and check to ensure. There were 5 staff on site. There was 1 client on site at the time of inspection.

LPA Nakagawa conducted interviews with Staff (S1) and other staff (S2) regarding the incident reported on 03/15/2026 regarding client C1. Staff (S2) stated that C1 came back from an outing and made a request for food. S2 stated that they only asked C1 to wait; there was no denial of the request.

LPA conducted an inspection of C1's room found it to be clean and well-organized. LPA had a discussion with Administrators regarding the care and behavior plans of C1 and C1's access to particular items. LPA requested records and documents.
