Facility Evaluation Report

On 07/28/2026, Licensing Program Analyst (LPA) Nakagawa arrived unannounced to conduct a case management and met with Administrator Mary Felix.

LPA is conducting the case management to review a recently submitted incident report involving three residents (R1, R2, R3). LPA reviewed facility resident files and obtained information regarding the incident. Administrator addressed the incident as required. All responsible parties were contacted, all residents involved were assessed, and changes to care plans were initiated, with no further incidents or behaviors occurring. All reports were completed and submitted as required.
