Facility Evaluation Report
On 01/23/2025 Licensing Program Analyst (LPA) Araceli Canela arrived at this Day Program unannounced to conduct a Required-1 Year inspection. LPA met with Kristi Lamb, Program Manager and explained the purpose of the visit. This Day Program is licensed for up to 35 participants of which 10 may be non-ambulatory in suites D&E. During todays inspection there were 11 participants and 9 staff present.
LPA Canela and administrator toured the facility together to ensure the health and safety of participants in care. Areas toured include but are not limited to the 2 section room identified as suite E and suite D which are made up of common areas, kitchen, storage area and 2 bathrooms. In the areas toured no immediate health, safety, or personal rights violations were observed.
The common area was clean and in good repair. The kitchen and bathrooms were clean and in good repair. Cooking/dining equipment and utensils were present. The facility was observed to be at a comfortable temperature. Smoke detectors are all operational. Hot water temperature measured within required Title 22 regulations of 105 degrees F and 120 degrees F. All employees requiring background checks are cleared. There is a schedule of activities Planned for the clients. All required postings are displayed within facility. Facility participant files and staff files were reviewed and in compliance. Medication was reviewed.
No firearms are on premises. The last disaster drill was conducted and documented on 12-26-2024. Fire extinguisher fully charged and serviced on 1/31/2024. This facility has fire sprinklers that are serviced yearly.
The facility is in compliance. No deficiencies are being cited as a result of today’s inspection. Exit interview conducted and copy of the report emailed to administrator.
