Facility Evaluation Report
06/16/2026 Licensing Program Analyst (LPA) Loera arrived unannounced to conduct an annual Required-1 year inspection and met with Administrator, Lamaya Lee. Facility is licensed for 6 ambulatory clients. There are currently no clients in care.
LPA and Administrator toured the building and grounds and observed the home to be clean at a comfortable temperature with all exits free from obstruction.
Smoke detectors and carbon monoxide detectors were found to be operational. Fire extinguisher located in the kitchen was observed to be charged. Bedrooms were observed with the required furnishings with additional bedrooms ready for when facility receives their first client. Water temperature in sinks were measured between 105 and 120 degrees F. Chemicals were found to be secured in a cabinet located in the kitchen.
LPA went over requirements once facility receives their first client, at this time individual, identified as S1 is associated and living at the facility.
No deficiencies cited during today's inspection. Updated copies of the following documents were requested and are to be submitted to CCL by 07/16/2026:
