Facility Evaluation Report

At approximately 9:20AM, Licensing Program Analyst (LPA) Magdaleno arrived at this facility announced to conduct a case management visit in regards to the closure of this facility and met with Licensee George Portan. At approximately 9:35AM, Licensee and LPA conducted a walk through of the building. LPA observed the home to be empty of client belongings and there were no clients present. LPA was informed by Licensee that the facility had not received clients. Licensed was forfeited to LPA.

Based on observation, there are no clients present and no one is receiving care or supervision.

Exit interview conducted with Licensee, whose signature on form confirms receipt.
