Facility Evaluation Report
On 03/26/2026, at approximately 9:40 AM, Licensing Program Analyst (LPA) Ali Deniz arrived announced to conduct a required 1-year annual inspection and was greeted by George Porter, Administrator. Facility is an Adult Residential Facility (ARF) with a capacity for four (4) clients. Facility has an approved fire clearance for four (4) ambulatory residents with no clients in care at time of visit. The facility is currently determining whether to retain the licensee, as North Bay Regional Center (NBRC) has declined the vendorization application.
At approximately 10:00 AM, LPA initiated a tour of the facility with Licensee and observed the following: Facility is a two (2) story home, was a comfortable temperature, and passageways were free from obstructions. Facility's fire extinguisher was observed charged and purchased 02/26. Smoke and Carbon Monoxide detectors were tested and operational during inspection. Water temperatures in client bathrooms read at 109.5 degrees and 110.1 degrees which is within the allowable range of 105 to 120 degrees F per Title 22 regulations. Clients' bedrooms were inspected and observed to have all the appropriate furnishings as outlined in Title 22 regulations. Cabinets containing cleaning supplies and other items that could pose a risk were locked. There is a seating area in the backyard and an enclosed lounge with room for activities.

LPA obtained copy of updated LIC308 - Designation of Responsibility.
No deficiencies cited. Exit interview conducted with Licensee, whose signature on form confirms rece ipt .
