Facility Evaluation Report
04/25/2025 Licensing Program Analyst (LPA) Sarah Benson conducted an announced visit to the facility for closure and final walk through. No one was present at LPA's arrival. LPA Benson contacted the Administrator Erykah Timberlake Small by phone.
The purpose of this visit was to determine whether there were any residents in care. The Administrator reported the last two residents moved out on 4-7-25, no residents are living at Barkers.
LPA Benson observed piles of garbage at the front door of administrators residence and heard a dog barking inside. There were two cars that looked to be abandoned/not working in the driveway. The building that housed the residents front door was open. LPA Benson ask if anyone was home and no one answered. LPA Benson observed no residents in care.
LPA requested the facility license to be returned to licensing.
No deficiencies were cited during this visit. A copy of the report was emailed to the licensee for a signature.
Licensee will return a copy of signed report to licensing.
