Facility Evaluation Report

On 03/24/25 Sarah Benson, Licensing Program Analysts (LPA) arrived at the facility unannounced to perform a health and safety check. LPA Benson met with Juanita Potts Licensee and explained the purpose of the visit.

During the visit LPA Benson interviewed the Licensee and five residents inquiring how the residents were dealing with the 60-day eviction notice. Three of the residents stated they didn’t know where they are moving, and they don’t want to move from Siskiyou County area. LPA Benson informed the residents of facilities out of the county where the residents needs could be met. One of the residents reported they are moving on 3-27-25 and is looking forward to it. The other residents reported Siskiyou County Mental Health and Adult Protective Services have been working with them to find new housing.

No deficiencies cited. Exit interview conducted and a copy of the report was provided to Licensee Juanita Potts.
