Facility Evaluation Report

A teams meeting was held today, 3/14/2025 at 11:30am, via Microsoft Teams. Present at this meeting was Sacramento North Region Manager (RM), Alycia Rayner, Licensing program Manager (LPM), Lauren Crocker, and Licensing Program Analyst (LPA), Sarah Benson. Facility representatives included Licensee Juanita Potts, and Amber Tuller, Siskiyou County Mental Health Coordinator Dan Bonham and Sail House, INC., Licensee Chris Carey.

The following was discussed during today's meeting:

Licensee Potts’ intent for maintaining the facility license moving forward.

Licensee Potts has opted to close the facility and issue notices to the residents and their responsible parties.

Ongoing care and supervision provided to residents.

Licensee Potts to verify sufficient staff are present to meet resident’s needs.

Licensee agreed to coordinate with staff, residents, and Mr. Bonham and his team a meeting to discuss the closure plan, answer questions, and provide support to residents, if needed.

Facility staff will provide copies of the resident files requested by the LPA by the end of the day.

A follow up meeting will be scheduled to discuss the progress and provide support. An exit interview was conducted, and a copy of this report will be provided to the Licensee via email. A copy will be signed and returned to the LPA. The signature of the Licensee on this form acknowledges receipt of this document.
