The inspector’s account

On May 20 2026 Licensing Program Analyst (LPA) Sarah Benson arrived unannounced to conduct a case management visit. During today's inspection LPA met with Administrator Todd Harris.

LPA arrived to obtain further information regarding incident reports submitted on 03-22-26, 4-5-26 and 4-14-26. During today's inspection LPA interviewed Administrator and requested resident documents. LPA and administrator inspected the facility and medication room. In addition, LPA interviewed 1 staff and 1 resident in care. LPA attached 812's to report.

No deficiencies were cited during this visit. Department will follow up further if warranted.
