Facility Evaluation Report
On January 23, 2026, at 10:30 a.m. Licensing Program Analyst LPA Sarah Benson arrived at the facility to conduct an unannounced case management visit in response to an email and phone call from the administrator. LPA met with CEO Mark Montgomery and Administrator Todd Harris and explained the purpose of the visit.
The Staff and LPA toured the facility together. During the visit files were reviewed and interviews performed.
The Administrator stated the self reported Quality assurance errors will be corrected in a timely manner. The Administrator stated I will report to the LPA as corrections have been completed.
No citations were issued per Title 22 Regulations.
Exit interview conducted and copy of the report was left with the administrator.
