Facility Evaluation Report

A Case Management visit was conducted on July 17, 2025, by LPA Kayla Adkison, as during the investigation of complaint #59-AS-20250423134418, it was determined that the licensee/administrator did not request from the licensing agency an exception for two residents residing at the facility that had been diagmosed with Methicillin-Resistant Staphylococcus Aureus (MRSA).

Based on observations, interviews which were conducted and record review(s), the preponderance of evidence standard has been met, therefore the above allegation is found to be Substantiated. California Code of Regulations, (Title 22), is cited on the attached LIC 9099D.

Exit interview conducted. Appeal Rights and a copy of this report was provided to Sherrill Denny, LVN and Michelle Decoito, Business Office Manager
