On 4/29/26 Licensing program analyst Marisa Chiarelli arrived at the facility unannounced for a case management visit, LPA met with Chris Bell staff member and explained the purpose of the visit.
On 4/20/26 LPA Chiarelli received a call from Chris regarding the administrator at the facility. Cassie is no longer the administrator and Guy Berry will be taking over as administrator.
Chris said he will send over the documentation so LPA can update the administrator in the system to reflect the new administrator.
No deficiencies are being cited as of today's visit. Exit interview conducted and copy of report left with the facility.
