Facility Evaluation Report
On February 19 2026, Licensing Program Analyst (LPA) Sarah Benson arrived at the facility unannounced to conduct a case management visit. LPA Benson met with House Manager Hannah Hunt who phoned Administrative Director Crystal Love and Licensees and explained the reason for the visit. LPA Benson and staff toured the facility together to ensure the health and safety of residents in care.
The department is following up about an Incident Report (IR) sent by facility on 02-12-26 stating that a resident had a fall with an injury.
LPA Benson conducted interviews with Licensee, staff, the resident and performed a file review. (see attached 812 for interview).
At this time, this case is under review and the department will do follow up as needed.
No citations were issued per Title 22 Regulations.
An exit interview was conducted and copy of the report left at facility.
