Facility Evaluation Report
On 6-27-25 Licensing Program Analyst LPA Sarah Benson arrived at the facility unannounced for a case management. LPA Benson met with Administrator Crystal Monismith. The purpose of the visit is in regard to building good relationships with the neighbors and client behaviors. LPA Benson delivered a copy of the Good Neighbor Guide for Adult Residential Facilities.
1. Intervening before incident. Staff reported the house consultant and staff is working with the client. Staff reported having constant reminders to the client of appropriate behaviors in the community. Staff reported the clients understand the importance of the conversations.
2. Staffing: Staff reported Far Northern has put together a plan to have extra staff at night for approval.
3. Identify behaviors. Staff reported the client usually prefers to not leave the home. Staff reported previous client with behaviors in the neighborhood has moved, the facility has reported fewer incidents in the neighborhood.
4. Outline how staff plan on addressing behaviors. Staff reported the client is working with North American Mental Health bi-weekly for evaluations.
5. Long term placement of the residents. Staff reported staff and Far Northern are working with the client to help the client be successful with his emotions and behaviors.
6. How is Far Northern working with you and the client. Staff reported Far Northern has been very helpful. Staff reported FNRC has set up quarterly meeting with consultant.
