Facility Evaluation Report

On June 10, 2025, at approximately 2:00 PM, an informal conference was conducted with representatives of Sapphire Residential at the Chico Regional Office. The purpose of this Informal Conference meeting was to discuss current issues at facility. Present in the meeting were, Licensing Program Manager (LPM) Lauren Crocker, Licensing Program Analyst (LPA) Kayla Adkison, Licensee, Randy Leak, Administrator, Kiara Walker, Administrator,Chantelle Browning, Administrator Crystal Monismith and Lauren Leak, Human Resources. Licensee was informed this Informal Conference is a part of the Administrative Action process, and that further non-compliance may result in an elevation to a formal non-compliance conference, which could lead to a referral to the Department's legal division for possible revocation of license. The informal conference process was explained during this meeting. Issues discussed during the meeting were:

- an increase in complaints received in the last three months

- approximately 25+ law enforcement calls for service

- personal rights concerns with residents.

During the Informal Conference, the facility has stated they will do the following to achieve continued and substantial compliance:

- Creating a training plan for staff with a description of who will attend, when will classes be provided, topics being discussed.

- Creating neighbor and facility relationships and how to improve them.

-Relationship building with staff and clients

-LIC 500 for depicting hours for administrator at each of the licensee's facilities.

No deficiencies were cited during today’s Office Visit. Exit interview was conducted. A copy of the Office Visit was signed and provided to the Administrator.
