Facility Evaluation Report
On 11-17-25 Licensing Program Analyst LPA Sarah Benson arrived at the facility unannounced to conduct a case management visit in response to an incident report submitted by the facility on 8-29-25. LPA Benson met with Administrator Crystal Monismith and explained the purpose of the visit.
LPA Benson reviewed the document with the administrator. The administrator reported a staff member hired three days previous was antagonizing the clients behaviors. The Administrator stated on 8-27-25 I received a phone call and text from staff members stating a staff member was antagonizing a client C1, calling them a piece of shit, you fucking ass hole. The administrator reported I called the staff member and ask them to leave the house in order to prevent any more escalation of behaviors. The administrator reported the staff member refused to leave and kept antagonizing the client. The administrator stated I called back when the other staff reported the staffs refusal to leave. The administrator called again and told the staff member to leave the house. It was reported as the staff member was leaving the house they ask another client C2 to step outside and told the client, I know why you are here. It was reported that the staff member went to their car and notified the administrator that they quit.
LPA inquired what steps would be taken to prevent staff from abusing residents personal rights, "Personnel shall provide for the care and safety of persons without physical or verbal abuse, exploitation or prejudice ."
The administrator stated the staff member quit on the day of the incident, is not longer working at the facility. The administrator reported concern that the staff member may go work with other clients at other facilities.
The administrator is having a crises intervention training/personal rights for staff on 11-12-25 and 11-21-25.
No citations were issued per Title 22 Regulations.
Exit interview conducted and copy of the report left at facility.
