Facility Evaluation Report
On 06/17/2026 Licensing program analyst Marisa Chiarelli arrived at the facility unannounced to follow up on an incident that was reported on 1/26/26 this incident has been an ongoing concern. LPA Chiarelli met with administrator Crystal Monismith and explained the purpose of the visit.
Back in January of 2026 it was reported a resident had a microwave in their room that had been cutting the circuit breaker. A electrician has come out and determine that the microwave is a safety concern in that room and a safety concern to the facilities wiring, which left in the room and used could potentially cause a fire. Confirmed with resident that they have removed the microwave from their room and understand why it was a safety concern.
LPA Chiarelli and administrator toured facility and observed that the microwave has been removed from the residents bedroom and is no longer in the facility.
No deficiencies are being cited on today's visit and copy of report given to administrator.
