Facility Evaluation Report
On June 24 2026, Licensing Program Analyst (LPA) Sarah Benson arrived at the facility, unannounced to conduct a case management visit. LPA Benson met with Direct Care Staff (DCS) and explained the reason for the visit. The administrator Kiara Walker and House Manager arrived ten minutes later.
The department is following up to the Incident Report (IR) submitted by the facility on June 23, 2026 concerning a former staff member and a resident. LPA Benson and the administrator toured the facility.
LPA Benson collected documents: Staff list with telephone numbers and work schedules, termination letter, staff file, admission agreement, needs and service plan, physician reports and IPP's for residents.
At this time, this case is under review, and the department will do a follow-up as needed.
No citations were issued per Title 22 Regulations.
An exit interview was conducted and copy of the report left at facility.
