Facility Evaluation Report
On August 25, 2025, at 10:30 AM Licensing Program Analyst (LPA) Kayla Adkison arrived at the facility unannounced to conduct a Required 1-Year inspection. LPA met with Rebecca Eutermoser, direct care staff, and administrator Jazmen Huff and explained the purpose of the visit. During the inspection, (1) one staff was present. Two (2) clients were engaging in activities at an off-site licensed day program and two (2) clients were at their places of employment. Administrator Trisha Steele arrived approximately 30 minutes later and continued the inspection.
LPA Adkison and Administrator Huff toured thhe facility together to ensure the health and safety of residents in care. Areas toured include but are not limited to four (4) resident rooms, common areas, two (2) bathrooms, kitchen, storage areas and back yard. (4) Four staff and (4) four resident files were reviewed which contained all required documentation. Medications were observed to be a locked cabinet, rendering them inaccessible to clients in care.
All common areas, bathrooms, bedrooms, and kitchen were clean and in good repair. All bedrooms had required furniture, bedding, and lighting. Cooking/dining equipment and utensils were present. Facility has required (7) seven-day non-perishable and (2) two day perishable supply of food.
The facility was observed to be at a comfortable temperature. First aid kit fully stocked and ready for emergency use. Fire extinguisher fully charged. Smoke detectors and carbon monoxide detectors are all operational. Hot water temperature measured within required Title 22 regulations of 105 degrees F and 120 degrees F. All employees requiring background checks are cleared. There is a schedule of activities planned for the clients. All required postings are displayed within the facility.The last disaster drill was conducted and documented in July 2025, the facility has been conducted them monthly.
No deficiencies are being cited as a result of this inspection. A copy of this report was provided to Administrator, Trisha Steele.
