On May 15 2026, Licensing Program Analyst (LPA) Sarah Benson arrived at the facility to conduct an unannounced case management visit in response to an incident dated 4-22-26 and reported on 4-22-26 to licensing. LPA met with Manager Shirley Robichaux and explained the purpose of the visit.
LPA Benson reviewed the Incident report and interviewed the Manager and performed a file review. Interview is attached in an 812.
An exit interview was conducted, and a copy of the report was left with the manager. No deficiencies are being cited as a result of todays case management.
