Facility Evaluation Report
Licensing Program Analyst (LPA) Ruth Wallace conducted an unannounced Required 1- Year Annual Inspection Visit and met with Program Director. Prior to initiating the visit, LPA completed required COVID-19 testing protocols, and a daily self-screening questionnaire for symptoms of COVID-19 infection to affirm no COVID-19 related symptoms. All staff were observed to be wearing surgical masks. Facility entrance is equipped with proper COVID-19 signage and screening station. Facility has a mitigation plan in place should a COVID positive case occur. Facility has sufficient supply of PPE. LPA observed all clients to be socially distanced and wearing surgical masks.
LPA toured facility with program director. The facility offers seven classrooms, including a computer room, three bathrooms, two kitchens and office space. There is a patio area on the side of the building for use by clients. The facility is clean and in good repair, and has a locked storage area for cleaning supplies, disinfectants, and poisons. The facility has an area for staff and client records, first aid supplies, and a telephone. The facility provides a separate room for use by ill clients. The facility has an emergency disaster plan on site and it is posted near the exit. Snacks are made available to clients during the day and the facility serves lunches on Friday. Hot water temperature was measured at 113.4 degrees Fahrenheit in bathroom sink, which is within the required range of 105 to 120 degrees. LPA was able to review a current Fire extinguishers inspected 2/7/22 and smoke detectors are operational. LPA observed carbon monoxide detectors in the facility. Fire drill was conducted on 9/8/22. The facility has functioning smoke alarms and carbon monoxide detectors. The fire marshal has approved the fire safety inspection request.
LPA reviewed 4 client and 3 staff files, including criminal record clearances. The first aid kit was checked and is complete.
No deficiencies cited per California Code of Regulations, Title 22.
Exit interview conducted and a copy of report was provided to program director.
