On February 19, 2026 at 1:00 p.m. Licensing Program Analyst (LPA) Sarah Benson, arrived to open a complaint. During today's visit the facility was toured, records were reviewed and interviews were performed. During the facility tour the bath room had urine on the floor and smelled of urine. During the interview LPA requested the sliding door open because of the smell.
The facility shall be clean, safe, sanitary and in good repair at all times for the safety and well-being of clients, employees and visitors.
