Facility Evaluation Report
On May 7 2026, Licensing Program Analyst (LPA) Sarah Benson arrived at the facility unannounced to conduct a case management visit. LPA Benson met with Administrator Sian Eubank and explained the reason for the visit.
The department is following up to the Incident Report (IR) sent by facility on 03-07-26 stating that a staff had incorrectly given a residents medication. (see attached 812 for interview).
LPA inspected the facility and reviewed the medication and medication administration record (MAR).
An exit interview was conducted and copy of the report left at facility.
The following deficiencies were observed (See LIC 809D) and cited from the California Code of Regulations, Title 22, and California Health and Safety Code. Failure to correct the deficiencies may result in civil penalties.
