On 6-27-25 at 10:30am Licensing Program Analyst LPA Sarah Benson conducted an unannounced case management visit. LPA Benson met with Administrator Nessa Warren.

This visit is to confirm the ORDERS TO INDIVIDUAL FOR IMMEDIATE EXCLUSION FROM ALL FACILITIES. The Administrator has stated the named individual is not working in the facility, has no contact with clients and this employee is not allowed to be physically present in any residential facility licensed by the California Department of Social Services. LPA Benson verified the named individual has been disassiociated.

LPA Benson and Administrator toured the facility together.
