Facility Evaluation Report

On March 4, 2026 at 11:00 a.m. Licensing Program Analyst (LPA) Sarah Benson, conducted an unannounced visit and met with Administrator Brenda Bigler and Christian Herr.

On 3-2-26 an incident report was submitted by administrator Christian Herr reporting bedbugs in a mattress of a vacant room. It was reported staff removed all bedding and curtains, washed and dried and place inside a tied garbage bags. It was reported all furniture was moved out back. The Pest Control was notified. It was reported the other rooms were inspected with no sign of bedbugs. During today's visit the facility was toured. It was reported the two resident have had no bedbug issues.

The administrator stated the Pest Control visited and reported they have to order something and will call back. It was reported the Health Department was contacted. It was reported Health Department stated it is not necessary to report unless it is in two separate rooms and a doctors diagnosis.
