Complaint Investigation Report
LPA investigated the allegation, “Licensee allows unqualified staff to transport residents.” Based on interviews, staff indicated that all staff have a valid driver's license. Staff use the facility van or personal vehicle to transport clients to and from appointments or outings. LPA could not corroborate the allegation.
LPA investigated the allegation, “Staff do not take residents on community outings.” Based on interviews and records reviewed, staff indicated that clients are encouraged to go on community outings when safe and appropriate. Interviews indicated that they have not heard or seen staff deny clients outings. LPA could not corroborate the allegation.
LPA investigated the allegation, “Staff no not ensure the facility has a sufficient amount of food.” Based on interviews and observations, the facility has an adequate amount of food for the clients. Food supplies in the facility were adequate per requirement. LPA could not corroborate the allegation.
LPA investigated the allegation, “Staff are not providing adequate supervision to residents in care.” Based on interviews conducted it was stated that clients are not left alone at the facility or unsupervised for extended periods of time. Staff interviewed acknowledged that when the weather is nice, the clients are free to go into the side yard while staff observe. Interviews indicated there are usually two to three staff working on each shift to provide adequate supervision. LPA could not corroborate the allegation.
Based on interviews conducted and observations, the preponderance of evidence standards has not been met. Therefore, the above allegations are found to be UNSUBSTANTIATED. Findings that the complaint is Unsubstantiated means that, although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred.
An exit interview was conducted, and a copy of the report was provided.
