Facility Evaluation Report
On April 14 2026 at 10:00a.m. Licensing Program Analyst LPA Sarah Benson arrived at the facility to verify dates for an incident report dated September 4 2025. LPA Benson met with Resident Care Coordinator Vai Tuaau.
The incident report stated the incident occurred on September 21 2025 and was signed September 4 2025.
LPA Benson interviewed the Resident Care Coordinator to verify the date of incident.
Records were reviewed and staff verified the reported date of September 4 2025 was a type error. The date the report was written and submitted was September 22 2025 and incident occurred September 21 2025.
No citations were issued per Title 22 Regulations.
Exit interview conducted and copy of the report was given to Administrator.
