On 12-17-25 Licensing Program Analyst LPA Sarah Benson arrived at the facility unannounced regarding a case management concerning two incidents reports received on 09-04-25 and 11-25-25. LPA Benson met with Administrator Kristine Boban, and explained the purpose of the visit.
Interviews were performed and files were reviewed.
No citations were issued per Title 22 Regulations.
Exit interview conducted and copy of the report was given to Administrator.
