Facility Evaluation Report

On December 11, 2025, Licensing Program Analyst (LPA) Ivan Avila, conducted a case management visit regarding an incident that was reported to the Department on November 8, 2025. LPA met with Administrator Nessa Warren and explained the purpose of the visit.

The department is following up on an Incident Report submitted by the facility on November 8, 2025 concerning a client injury.

At approximately 8:30 AM on November 7, 2025, after C1 had a shower and breakfast, facility staff transported C1 to Day Program. Shortly after drop-off, the Administrator received a call from the Day Program reporting that C1 had redness on their stomach. The Administrator returned to the program, picked up C1, and observed the affected area. C1 was then taken to Shasta Regional Emergency Room for evaluation. The doctor checked out the area and prescribed a topical cream for treatment.

LPA obtain further information regarding C1. During today's inspection LPA interviewed Administrator and collected documents.

No deficiencies cited during today's inspection. Exit interview conducted and copy of the report was provided.
