Facility Evaluation Report
10/04/2022 11:00 AM Licensing Program Analyst (LPA) Rebecca Knight, made an unannounced visit to the facility and met with administrator Vanessa Warren. The purpose of this visit was to interview the administrator regarding an incident report that was submitted regarding a client that attends Bravo Access day program.
Prior to initiating the visit, LPA completed a daily self-screening questionnaire for symptoms of COVID-19 infection to affirm no COVID-19 related symptoms. LPA ensured they applied hand sanitizer before entering the facility and the following Personal Protective Equipment (PPE) was worn: N95 mask, gloves.
LPA interviewed the administrator and requested copies of the following documents: Staff list with phone numbers, Physicians documentation of visit for C1.
No deficiencies cited. Exit interview conducted and a copy of the report was emailed to administrator Vanessa Warren.
