On May 7 2026, Licensing Program Analyst (LPA) Sarah Benson arrived at the facility to conduct an unannounced case management visit to interview staff and residents concerning complaint 59-AS-20250912130709 submitted on 9-12-25. LPA met with Care Staff and was admitted into the facility. Care Staff notified the administrator of LPAs visit. Care staff and LPA inspected the facility. LPA met with Administrator Troy Watkins, Theresa Edwards, Amber Watkins and Kelly St.louis.
LPA Benson interviewed two staff, Administrator and one resident, performed a file review. Interviews are attached in 812s.
