The inspector’s account

On April 30 2026 Licensing Program Analyst (LPA) Sarah Benson arrived unannounced to conduct a case management visit. During today's inspection LPA met with Care staff Kelly St.Louis.

LPA arrived to obtain further information regarding an incident report submitted on 4-16-26. During today's inspection LPA requested resident documents inspected the facility. In addition, LPA interviewed 1 one staff and 1 one resident in care.

No deficiencies were cited during this visit. Department will follow up further if warranted.
