Facility Evaluation Report

On November 13, 2025, Licensing Program Analyst (LPA) Ivan Avila arrived at the facility unannounced to conduct a case management follow-up visit and met with Shelby Kane. Today’s visit is to ensure an individual who was issued an exclusion letter on November 6, 2025, isn’t working at the facility.

S1 cannot be allowed to work, be present and/or live in a CCL licensed facility or have contact with clients in any residential facility or child day care licensed by the California Department of Social Services. Therefore, the Department orders the facility to remove S1 from any contact with clients and not allow this employee to be physically present in the facility.

LPA explained that if S1 were present in the facility, S1 needed to leave immediately. S1 is to be removed from all shifts and disassociated from the facility in Guardian.

Assistant Director Shelby Kane stated S1 has not worked at the facility since August 2025.

No deficiencies are being cited as a result of today's visit. Exit interview was conducted. Signature on these forms acknowledges receipt of these documents.
