On November 19, 2025, Licensing Program Analyst (LPA) Ivan Avila arrived at the facility to conduct a case management visit. LPA met with Administrator Amber Buxton and explained the purpose of the visit.
LPA arrived to obtain further information regarding a resident. During today's inspection LPA interviewed Administrator and gathered information regarding an incident report that will be sent to the Department.
No deficiencies cited during today's inspection. Exit interview conducted and copy of report was provided.
