Facility Evaluation Report
On October 29, 2025, Licensing Program Analyst (LPA) Ivan Avila arrived at the facility unannounced to conduct a Case Management visit and met with Administrator Amber Buxton and explained the purpose of the visit. Also participating in this inspection are, LVN Suzanne Manning, California Department of Developmental Services Nurse Consultant, Cristie Vericker, and Far Northern Regional Center Nurse, Jennifer Badger. LPA Avila and Administrator toured facility together to ensure the health and safety of residents in care.
LPA conducted an inspection of the care home to ensure compliance with Title 22 regulations. There are five (5) bedrooms and three (3) bathrooms for resident use. LPA observed bedrooms to be properly furnished, with appropriate bedding and lighting. The bathrooms were in sanitary condition and properly maintained.
California Department of Developmental Services Nurse Consultant, Cristie Vericker conducted a medication audit and discussed medication documentation and the importance of following a cohesive system to limit medication errors in the future. LPA Avila spoke with Administrator regarding medication errors that have occurred at the facility. LPA explained to the Administrator on the importance of ensuring that all medications are dispensed as outlined in the resident’s physician orders and as outlined in Title 22 Regulations. LPA also advised that if another medication error happens again, a citation will be issued.
Several topics were discussed that include but not limited to: staffing, labor management, resident rights, staff training and Title 22 regulations. LPA Avila also provided the Full Bed Rail Waiver and PRN Waiver to Administrator Amber Buxton.
As a result of today's inspection, no deficiencies are being cited. Exit interview was conducted and a copy of this report was provided. Signature on these forms acknowledges receipt of these documents.
