The inspector’s account

On 9-22-25 at 1:00PM a Non-Compliance Conference meeting was conducted via TEAMS.

Present in the meeting today were Regional Manger Alycia Rayner, Licensing Program Manager Lauren Crocker, Licensing Program Analysts Sarah Benson. Merakey Old Alturas Executive Director, Brandi Auble, Director of Programs, Cinthia Rubio, Executive Director of Development and Programs Robyn Cheskiewicz and Administrator Jessica Boarman.

Issues discussed during the meeting were:

Staff training, Supervision, Reporting abuse, Gap in reporting, Chain of command, Steps for reporting, Completion of SIR, SOC 341, What policies were reviewed at training submitted for POC, Office time, Listening and responding appropriately.

A compliance history was also discussed to include the facility's citations for the past 3 yea rs.

Dates of implementation and staff training's

The 9099D dated 09-04-25 will be amended to reflect an immediate five-hundred-dollar civil penalty assignment that will be issued today per California Code of Regulations, Title 22, Division 6, Chapter 8.

An exit interview was conducted and a copy of this report and appeal rights will be provided to the facility via email.

A copy will be signed and returned to CCLD.
