Facility Evaluation Report

Licensing Program Analysts (LPA) Sarah Benson arrived at the facility at 4:00pm to conduct an unannounced case management visit on 07/10/2025. LPA Benson was met at the door by lead care staff Bailey Ashby and given permission to enter the facility. This visit is to confirm ORDERS TO INDIVIDUAL FOR IMMEDIATE EXCLUSION FROM ALL FACILITIES.

LPA met with Administrator Jessica Boarman. and stated the purpose of visit. The staff understands this is an Immediate Exclusion effective 07/10/2025 and S1 is excluded and cannot be allowed to work, live in, and/or have contact with clients in any residential facility licensed by the California Department of Social Services. Therefore, the Department orders the facility to remove S1 from any contact with clients and not allow this employee to be physically present in the facility.

The Administrator stated S1 has been on administrative leave since 6-20-25 and the last day they worked was 6-16-25.

Exit interview conducted, a copy of this report was signed and given to the Administrator.
