LPA Hiratsuka, conducted this unannounced plan of correction visit. The citation was issued on 05/03/2023, during an office meeting conducted over Microsoft Teams. The facility was cited for not submitting incident reports.
Today, LPA obtained a written statement stating they have submitted all the missing incident reports to LPA Benson via email, the staff were trained on incident reporting, and their system has been updated to include Community Care Licensing when submitting incidents.
