Facility Evaluation Report
On 01/23/2026, LPA Marisa Chiarelli arrived at the facility unannounced for a case management visit. LPA Chiarelli met with Regional Director Carrie Lustig and went over the incident that occurred on 01/01/2026. 01/12/2026 a SOC 341 was received from the facility about an incident that involved a staff member who was verbally harassing a resident.
Employee involved in the incident was placed on suspension effective immediately following the incident and officially was terminated from employment as of 01/14/2026. Interviewed resident involved in the incident and has been doing well since the incident occurred.
No deficiencies were cited on during today's visit. Exit interview conducted, copy of today's report given to Carrie.
